Abstract: With the number of people affected with AIDS increasing at an alarming rate--by December 1986 there were almost 30,000 cases in the United States and over 600 cases in the United Kingdom--the epidemic remains a phenomenon of unusual importance and an intense research effort is in progress. Worldwide, the AIDS pandemic now affects over 80 countries. It is estimated that for every AIDS patient (pt) there are about 50 other persons infected with the AIDS virus. It has been well established that AIDS is caused by a virus called human immunodeficiency virus (HIV), which can be transmitted by sexual contact, by infected blood via infected needles and syringes, from infected mother to unborn child, or by treatment with contaminated blood or blood products. About 30,000-100,000 people in the UK and 1-1.5 million in the US are estimated to be infected with HIV, and experts predict that 30-100% of them will eventually develop fully expressed AIDS. This second edition on AIDS presents updated information on the following topics: epidemiology of AIDS (the evolution of AIDS, tracking the epidemic, distribution of AIDS, UK public health regulations related to the spread of AIDS), the risk groups (homosexual or bisexual men, iv drug abusers, hemophiliacs, other recipients of blood products, heterosexual partners of AIDS pts, childhood AIDS, cases associated with Central Africa, Haitians), the cause of AIDS (hepatitis B virus, cytomegalovirus, Epstein-Barr virus, retroviruses, HIV, cofactors involved in HIV infection), transmission of the AIDS virus (sexual contact, parenteral transfer, perinatally [vertical transmission], salivary transfer, incubation period of the AIDS virus), infection with the AIDS virus (tests for the AIDS virus, HIV infections, recommendations for the individual with HIV infection, advice for general practitioners handling AIDS testing), early symptoms and signs of AIDS (persistent generalized lymphadenopathy [PGL], AIDS-related complex [ARC], evolution of PGL and ARC to AIDS), AIDS clinical picture (definition of AIDS, spectrum of disorders seen in AIDS, clinical features of AIDS, new classification for HIV infection), Kaposi's sarcoma (definition, Kaposi's sarcoma and AIDS), and treatment and management of AIDS (treatment of opportunistic infections, of Kaposi's sarcoma, of the immune deficiency; anti-HIV agents--inhibitors of virus replication; other treatment; general management and precautions when dealing with AIDS pts; counseling AIDS pts). Also included are a glossary; information for blood donors; AIDS--WHO consultation; cellular immunity; definition and revision of the case definition of AIDS used by the Centers for Disease Control, case definition of AIDS in children; general background on viruses; UK hemophilia reference centers and regional centers of the National Blood Transfusion Service in the UK; and selected bibliography and useful addresses.
