Abstract:
The background of chemotherapy in non-AIDS-related Kaposi's sarcoma (KS) is summarized, agents that have been used in patients (pts) with AIDS are discussed, and management guidelines are offered. The following therapeutic agents are discussed: vinblastine, vincristine, etoposide (VP-16), alternating vinblastine and vincristine, combination chemotherapy, alpha-interferon (IFN), and miscellaneous agents. Guidelines for therapy in AIDS-related KS are as follows: (1) for minimal tumor extent with slow progression and favorable prognostic factors--no treatment first, then vinblastine-vincristine or alpha-IFN as a second option or, for pts with unfavorable prognostic factors, vinblastine-vincristine or alpha-IFN first and no immediate treatment as a second option; (2) for extensive disease with rapid growth and favorable prognostic factors--etoposide, Adriamycin, or vinca alkaloids as a first option and alpha-IFN as a second option or, for pts with unfavorable prognostic factors, etoposide or Adriamycin first and vinca alkaloids second; and (3) for pts with locally symptomatic lesions--radiation therapy.
