[ACQUIRED IMMUNODEFICIENCY SYNDROME AND KAPOSI'S SARCOMA OF THE RECTUM IN A HOMOSEXUAL MAN. APROPOS OF A CASE IN RIO DE JANEIRO]

Abstract:
A 25-yr-old bisexual man presented with rectorrhagia which he attributed to hemorrhoids. A rectal tumor was found which proved to be a Kaposi sarcoma. Hyperpigmented, pruriginous lesions were also found on the back and forearms. Palpable lymph nodes were felt in the neck, axilla and inguinal region. The patient (pt) also tested positive for syphilis and hepatitis B markers. He denied having travelled outside Brazil, and he admitted to occasional use of marihuana but not intravenous drugs. The pt's immune system was hyporesponsive to skin tests and his helper:suppressor ratio was 0.78. Lymphocyte activation in response to mitogens was also deficient, even when compared with other Kaposi's sarcoma pts without AIDS. These data confirmed that the pt suffered from Kaposi's sarcoma secondary to AIDS, and he was treated with levamisole (2 mg/kg for 3 days in six 15-day cycles) resulting in less than 50% tumor reduction. Alpha interferon was administered (10,000,000 units/m2 im for 28 days). The rectal tumor responded completely to this treatment: two subsequent biopsies demonstrating no residual tumor.
