Abstract: The immunomodulatory effects of indomethacin (25 mg/day qid for 4 wk) were studied in 8 patients with acquired immune deficiency syndrome (AIDS)-related complex (ARC) and 8 patients meeting the Centers for Disease Control surveillance definition of AIDS. There was no significant effect of indomethacin treatment on mean OKT4/OKT8 ratios and mean mitogen-induced lymphocyte proliferative responses (measured as [3H]thymidine incorporation) in these patients. However, the OKT4/OKT8 ratio increased upon treatment with, and decreased after discontinuation of, indomethacin in three patients with ARC and one with AIDS. Similarly, indomethacin treatment was associated with improved lymphocyte proliferative responses to phytohemagglutinin in two patients with ARC and four patients with AIDS, to concanavalin A in one patient with ARC and four patients with AIDS, and to pokeweed mitogen in one patient with ARC and one patient with AIDS. These lymphocyte proliferative responses increased markedly after 28 days on indomethacin and decreased at 14 days following discontinuation. Clinically, three patients gained weight, and improvement was found in one with diarrhea, two with Candida infection, and one with lymphadenopathy. The improved mitogen-induced lymphocyte proliferative responses and clinical improvement observed in some of these 16 patients indicate that further trials appear warranted to establish immunologic effectiveness and clinical benefits.
