Abstract:
The preinvasive stage of many carcinomas arising in squamous or glandular epithelia is well established, but the precursor lesions of nearly all soft-tissue sarcomas are unknown. As a rule, benign soft-tissue tumors do not become malignant. The author was aware of only a few sarcomas that arise in preexisting benign lesions. For instance, malignant schwannomas may be a late complication of neurofibromatosis; lymphangiosarcomas appear almost exclusively in long-standing chronic lymphedema; certain types of sarcomas develop in previously irradiated tissues, and Kaposi's sarcoma has been observed in immunosuppressed individuals. At present, it is not possible to predict from either clinical or morphologic grounds those patients who will go on to develop a malignant tumor. However, close follow-up of these high-risk patients may lead to the detection of small malignant tumors, especially those located in superficial soft tissues. Further details are presented under the following headings: neurofibromatosis and malignant schwannoma; Gardner's syndrome and fibromatosis; chronic lymphedema and lymphangiosarcoma; lipoma and liposarcoma; postradiation sarcomas; previous surgery, chronic inflammation, and malignant fibrous histiocytoma; and immunosuppression and Kaposi's sarcoma.
