Abstract: A case of acquired immunodeficiency syndrome (AIDS) is reported. The patient was a 45-yr-old male homosexual who presented with wt loss, diarrhea, cough, and dyspnea. Physical examination revealed fever, dehydration, oral and genital herpes, decreased breath sounds in the right lower lung field, and epigastric tenderness. The hematocrit was 32.3%, and the arterial oxygen saturation was 77%; other serologic and microbial tests were negative. Chest radiography showed an infiltrate in the right middle lobe, but bronchoscopic washings and transtracheal biopsy were negative for bacteria or fungus. The T4/T8 ratio was 53%. The patient required mechanical ventilation; open lung biopsy demonstrated the presence of Pneumocystis carinii. Forty-five days after presentation, the patient died of respiratory failure and herpes sepsis. Clinical aspects of AIDS are discussed. (14 Refs)
