[ACQUIRED IMMUNE DEFICIENCY SYNDROME: A CASE REPORT]

Abstract:
A case of Acquired Immune Deficiency Syndrome (AIDS) is reported. The patient was a 35 year old white Brazilian man who presented in May, 1982 with diarrhea for 1 month and weight loss. History was significant for homosexual relations; for generalized lymphadenopathy 2 years previously, diagnosed and treated as tuberculosis; and for interstitial pneumonia 1 year previously, treated successfully with antibiotics. Physical examination was significant for cachexia and cutaneous lesions consistent with generalized pseudoicterus. The hematocrit was 35% and the leukocyte count was 2,300/mm3, with 5% blasts, 73% segmented forms, 18% lymphocytes and 4% monocytes. Stool samples were positive for Strongyloides. The patient gradually became unresponsive and developed generalized seizures, at which time the leukocyte count was 58/mm3. Computerized tomography of the head revealed multiple enhancing lesions of the left cerebral hemisphere and a mass effect. The patient developed Klebsiella and Pseudomonas pneumonia and then sepsis, to which he succumbed despite aggressive therapy. Autopsy demonstrated multiple cerebral nodules that contained Reed-Sternberg cells and were felt to be of reticuloendothelial origin; Candida esophagitis; Strongyloides pulmonary abscesses; extensive cytomegalic inclusions; and pneumonia positive for Candida, Pneumocystis carinii, gram negative and gram positive bacteria, and Actinomyces. The authors conclude that this represented a characteristic case of AIDS, and that the patient's lymphadenopathy and interstitial pneumonia were early signs of the syndrome. (22 Refs)
