STAGING AND TREATMENT OF KAPOSI'S SARCOMA IN PATIENTS WITH AIDS

Abstract:
Kaposi's sarcoma (KS) is classified as Stage I when cutaneous and locally indolent, Stage II when cutaneous and locally aggressive, Stage III when mucocutaneous or when lymph node involvement is generalized, and Stage IV when visceral. The disease is subtyped as A when systemic signs or symptoms are absent and as B when they are present. Complete staging requires skin photography and biopsy, abdominal computerized tomography, endoscopy, colonoscopy, bronchoscopy (when the chest radiograph is abnormal), liver scan, and bone scan (when the alkaline phosphatase is abnormal). Estimated probability of survival at 36 mo has been reported to be 100% for Stage I, 60% for Stages IIIA and IVA, and 20% for Stages IIIB and IVB. In a nonrandomized clinical comparison of etoposide and combination therapy with adriamycin, bleomycin and vinblastine (ABV), complete responses were observed in 37.5% and 18.5% of etoposide and ABV patients (pts), respectively; partial responses in 40.6% and 62.9%; and no responses in 16.6% and 18.5%. Opportunistic infections have been more frequent with the ABV. Treatment of 5 cases with vinblastine alone resulted in partial response in 3, minimal response in 1, and no response in 1. The author concludes that these trials represent a promising beginning in the treatment of KS itself, but that treatment of immune dysfunction may be more important for pt survival. (7 Refs)
