Abstract: In a demographic analysis of risk factors for Kaposi's sarcoma (KS) among homosexual men, 20 of the first 21 men with biopsy-proven KS seen at the New York University Medical Center were compared with 40 age- and race-matched homosexual controls. The subjects were interviewed by questionnaire regarding sexual practices in the yr before diagnosis of KS. No significant differences between KS cases and controls were observed for age, race, ancestry, religion, birthplace, or residential history. Duration of homosexual activity was 17.5 +/- 7.7 yr for the KS cases and 15.0 +/- 7.3 yr for the controls. Relative risks of KS were 1.3 for active oral-genital intercourse (OGI), 1.5 for passive OGI, 1.9 for passive OGI with partner ejaculation, and 2.7 for passive OGI with swallowing of partner ejaculate; the risks were 1.2 for active anal-genital intercourse (AGI), 4.6 for passive AGI, and 11.3 for passive AGI with partner ejaculation. Multiple logistic regression analysis demonstrated a relative risk of 8.1 for passive AGI with partner ejaculation and 2.0 for lifetime use of amyl nitrites; the author concluded that these two practices are independent risk factors for KS in homosexual men. Enhancement by amyl nitrites of the absorption of semen or a semen-borne agent is hypothesized, and the use of condoms during sexual activity with persons at risk for the Acquired Immune Deficiency Syndrome is recommended.
