Abstract: Among conditions that may follow infection with Epstein-Barr virus (EBV), is malignant lymphoma in males affected by the X-linked lymphoproliferative syndrome (XLP). To test the hypothesis that the variable expression of XLP is manifested in obligate carrier females, blood from 12 obligate carrier females was studied for anti-viral capsid antigen (VCA), anti-early antigen (anti-EA) and anti-Epstein-Barr nuclear-associated antigen (EBNA). All carriers were seropositive to EBV. The reciprocal geometric mean titer for IgG anti-VCA was elevated to 570 in the carrier females compared to 100 in 24 controls. The range of anti-VCA was from 80 to 5,120. IgA class anti-VCA was found in 2/3 of the carriers. All but one of those with anti-VCA of IgA class showed anti-EA, indicative of active infection and immunodeficiency. About one-half of the carriers had persistent anti-EA antibodies. However, the anti-EBNA levels of carrier females were similar to those of the healthy controls. There was no apparent relationship between IgM level and EBV-specific antibody titers. The paradoxically elevated EBV-specific antibodies which tend to persist in obligate carriers of XLP provides an opportunity to define the immunopathology of XLP and to detect carrier females.
